How to Enroll

Participating in the EPR requires very little of your time and only a small amount of blood (20 ml, which is slightly more than 1 tablespoon). At enrollment, you also will be asked to provide us with your name, address and contact information, and provide your race, ethnicity, date of birth, and gender. You will receive $20.00 for participating in the EPR to compensate you for your time and travel expenses.

After this initial appointment, you will be contacted every year and asked to update your contact information. You will receive a recontact card in the mail. Please review the card carefully. If the information on the card is correct, just check the box stating so. If the information is not correct, then please update your contact information in the spaces provided on the back of the card.

When you receive this card, we will also ask you to provide two alternate contacts. Alternate contacts would be the names and numbers of one or two people who do not live with you, but who usually know how to contact you, such as a relative, close friend, or neighbor. We would only contact these individuals if we are having trouble contacting you for yearly updating or to ask you to participate in a follow-up study. There is space provided for alternate contact information on the recontact card we will send you each year. Providing alternate contact information is NOT a requirement for participation, and is completely voluntary.

Once you complete your annual recontact card, please return it using the postage-paid (pre-stamped and pre-addressed) envelope provided. Updating your contact information each year is very important because it ensures we are able to contact you to participate in voluntary follow-up studies.

Would You Like to Host an EPR Recruitment Event/Study Drive?

If you are a corporation, church, community group or other organization of any type, and would like to host an EPR recruitment event/study drive, please contact Polly Armsby (see below). If your organization holds health employee health fairs, it would be a good opportunity for employees to learn more about environmental diseases and participate in research.
